If you are visiting us for your first appointment, why not download our forms and print them off at your convenience? Please fill in as much information as you can, or if you have any questions, please call us at (403) 543-7499 and ask us for help. Simply bring your completed forms to your appointment. Fees
Alberta Health Care subsidizes $13.61 per visit up to a maximum of $200.00 per year, July to July.
The patient portion can be fully covered by extended insurance available through some policies. We recommend contacting your extended insurance company to confirm the details of your policy.
Please note that we do not bill secondary insurance companies on your behalf, but we would be happy to assist you with your individual insurance forms.

PARKING
Please note that parking in the Kensington area is constantly monitored.
- Metered Parking on Kensington Road Rate: $0.25 per 10 minutes
- Underground Parking at the rear of the building on 11A Street.
Rate: $8.00 per day
- 2 Hour Parking around St. John's Elementary School on 12th Street.
Rate: Free for 2 hours only
BUS ROUTES
- 1 Forest Lawn/Bowness
- 405 Sunnyside Community Shuttle
Both buses stop directly in front of the Kensington Professional Centre. Other numerous buses stop on 10th Street NW near Kensington Road.

HANDICAP ACCESS
There is a handicap access door at the side entrance of the building on 11A Street.
